Heart Failure Task Force
The Heart Failure Task Force vision is to decrease the morbidity and mortality associated with heart failure across mid-Michigan.
The following three bullets outline the Heart Failure Task Force outcome options:
- To improve compliance with American College of Cardiology and American Heart Association practice guidelines for the care of heart failure patients;
- To improve heart failure quality of care measures;
- To lower the risk of death and 30 and 60-day re-hospitalization in patients hospitalized for heart failure.
Outlined below are the nine hospitals across mid-Michigan that are participating in this guideline adherence initiative:
