The Diabetes Group Visit Project seeks to develop strategies to expand the utilization of clinical diabetes group visits throughout Genesee County provider organizations while tracking outcomes and effectiveness. The Project also aims to reduce the morbidity related to diabetes and diabetes related illnesses by improving the quality of life of individuals with diabetes. This Project places special emphasis on patient and support system interaction, adherence to drug therapies, patient education, and improved self-management and understanding of the disease and its long-term complications.
Through this effort, the Project’s Workgroup will develop strategies for the expansion of the Diabetes Group Visit concept to other healthcare organizations within Genesee County, including McLaren Regional Medical Center, Genesys Regional Medical Center, Hurley Medical Center, Hamilton Community Health Network, and any other potential Genesee County organizations or physicians exhibiting significant capabilities and/or interest.
A comprehensive Diabetes Group Visit Project “Replication Manual” has been developed with a portion of grant funding support from the Community Foundation of Greater Flint. This document will be continually refined and updated as the GFHC continues its efforts to expand the use of diabetes group visits throughout the community.
